'Patch' is a synonym for 'preset'. It's the specification of stored settings that define one tone/sound that is instantly recallable for playing on your device. For instance, the Pod HD500 stores 512 patches/presets organized into 8 setlists of 64 presets (16 banks of 4, from 1A through 32D). Each one of these patches specifies the selected amp, FX, inputs, mixer settings, etc that determine the sound of the patch/preset.
