HEADPHONE OUT - Anyone know the opamp required to fix it? No resources in the usual places

I must have got a lemon. I had the phantom power fail when I had a dynamic plugged in, engaged the p48 (accidentally but seriously should not have affected it...45 years of studio experience, even accidentally plugging ribbons and NEVER one issue) and it immediately died. So Im about to fix that...such a serious design flaw...check the net for the amount of complaints. 

 

NOW

 

Im listening through cans at times and the right channel starting crackling in and out...then left channel only. Yeah Yeah...Ive read a heap of articles and it would appear to def be the opamp as its not uncommon AS WELL!

 

Does anyone know the chip or replacement resource?
